Math Achievement Program Application

Complete this application to to apply to the Mathematics Achievement Program at the Samueli School of Engineering. The deadline to submit your application is Nov. 27, 2021 by 11:59 p.m. PT

ALL APPLICANTS ACCEPTED TO THE MATHEMATICS ACHIEVEMENT PROGRAM (MAP) MUST BE FULLY VACCINATED FOR THE COVID-19 VIRUS TO PARTICIPATE IN THE PROGRAM. EVEN IF ADMITTED TO MAP, AN UNVACCINATED STUDENT WILL NOT BE ABLE TO PARTICIPATE IN THE PROGRAM.

All applicants must meet the following criteria to apply to the program:

1. Students currently enrolled in the 9th grade
2. Completion of the following math classes by June 2022:
   • Geometry
   • Will consider excellent candidates enrolled Algebra 1
3. Passed the most recent math class with a B or better
4. Overall GPA = 3.0 or above

Program Dates: Jan – May 2022 (Dates: Jan 15, 22, 29, Feb 5, 12, 19, 26, Mar 5, April 2, 9, 16, 30, May 7)

Times: Saturdays 9:00 a.m. – 12:00 pm OR if students participate in a workshop of field trip day will be from 9:00 – 3:00 p.m.

Place: University of California, Los Angeles Campus

Classes: (subject to change)

• Morning Class: Algebra 1/Geometry Enrichment
• Afternoon Class: Workshop, Lab Tour, Project

IMPORTANT NOTE: If the application received by the CEED Office is not complete, you WILL NOT be allowed to submit missing items. Your application will be disqualified and WILL NOT be considered for admission to the 2021 Mathematics Achievement Program.

1. Complete application packet must be SUBMITTED before 11:59 p.m. on 11/22/21.
2. A complete application packet MUST contain all of the following items:
   • Completed and signed application form
   • A One-page essay based on the 4 questions provided (please make sure to have an adult proofread your essay before submission)
   • One teacher recommendations using forms provided (Recommendations MUST be from a math teacher whose class you have completed or are currently enrolled)
   • Required documents: 8th Grade Transcript, Proof of Free /Reduced Lunch (if qualified), completed MAP Policy
3. Students will be notified of admission status by Nov. 30, 2021. Admitted students and parents are required to attend the MAP Orientation. Orientation will occur during the week of Dec. 11, 2021 in the evening.

Students will be admitted to the program based on the following criteria:

1. 30% - Student’s Statement of Purpose (Essay Questions)
2. 25% - Academic preparation and performance
3. 20% - Teacher Recommendations
4. 25% - MDTP Results/Math Grades

Students applying to the MAP will take a MDTP Geometry Readiness test on Saturday, Nov 20, 2021 at 9:00 am. The length of the test is 45 minutes. Additional information will be provided after the MAP application is submitted.
